| Elmore now offers three options to submit an adoption application. Use the method that best suits your needs. |

- Option 2 - Electronic fill-in PDF file
Must have Adobe Reader 8 or newer (free download) to type, save, and print changes to downloaded file.
Download, open file, type information in form, save and email, or print copy.
Earlier Reader verisons and browser pdf form fill-in options will only allow typing of information and printing, but will not save typed information.

ADOPTION PROCESS |
- Identify what you are looking for in a pet.
|
- Fill out an application. Pre-applying allows you to get preapproved for adoption, then when the pet you have been looking for becomes available, you'll be ready without delays.

- Review our PetFinder listings.
|
- Visit the shelter, talk to our knowledgeable staff.
|
- Keep in touch, we want to know how it is going. Let us know if you have questions.

| DOG ADOPTION FEE INCLUDES: |
Spay/neuter, distemper vaccine, rabies vaccine (3 months and older), heartworm test, nails clipped, ears cleaned, teeth cleaned, de-flea treatment

| CAT ADOPTION FEE INCLUDES: |
Spay/neuter, FeLV/FIV/heartworm testing, rabies vaccine (3 months and older), distemper 5 in 1 vaccine, ear mite treatment, flea treatment
